I want to receive Jesus Christ

I'd bet that choosing that last link was one of the hardest decisions of your life... nothing on this page is any more difficult. Consider what Jesus taught,

"Ask, and it will be given to you; seek, and you will find; knock, and it will be opened to you. For everyone who asks receives, and he who seeks finds, and to him who knocks it will be opened. Or what man is there among you who, if his son asks for bread, will give him a stone? Or if he asks for a fish, will he give him a serpent? If you then, being evil, know how to give good gifts to your children, how much more will your Father who is in heaven give good things to those who ask Him!" (Matt 7:7-11 NKJV)

Paul wrote:

"...the gift of God is eternal life in Jesus Christ our Lord.” (Romans 6:23b)
"...if you confess with your mouth, ‘Jesus is Lord’, and believe in your heart that God raised Him from the dead, you will be saved… Everyone who calls upon the name of the Lord shall be saved.” (Romans 10:9,13)

You probably know that asking and calling upon God is called prayer. So all you have to do to receive God's gift is to ask Him, or call upon Him in prayer. The act of prayer is evidence of believing with the heart.

There isn't a set format for praying to receive God's gift of eternal life, just talk to God. Tell Him what's on your heart. But, if it's helpful, you can use the following as a guide:

If we receive the witness of men, the witness of God is greater; for this is the witness of God which He has testified of His Son. He who believes in the Son of God has the witness in himself; he who does not believe God has made Him a liar, because he has not believed the testimony that God has given of His Son. And this is the testimony: that God has given us eternal life, and this life is in His Son. He who has the Son has life; he who does not have the Son of God does not have life. (1 John 5:9-11 NKJV)
